All forms should display in your browser. Fill in the form then use your print function to get a printed copy of the form with the information you have entered. All forms should fit on 8.5 by 11 paper. You may need to change your margins in page setup. Also when you print select the pages option and select pages 1 to 1. Multiple blank pages may print if you do not select this option.

Send payment to the CDC Office
We encourage you to put money on your CDC account for future processing
Completed FORMS will be retrieved daily by the CDC

Registration Rate Increase
As of January 1, 2009 the registration rates have been increased as follows:
-CDC Member Sheep Under 1 yr of Age: $6.00
-CDC Member Sheep Over 1 year of Age: $7.00
-CDC Non-Member Sheep Under 1 year of Age: $8.00
-CDC Non-Member Sheep Over 1 yr of Age: $9.00
ALL Rush rates are doubled.
